To be eligible for medical case management services, you must reside in Lake, Orange, Osceola, or Seminole Counties. You cannot have the same or similar services through Medicaid or insurance. You must be at, or below, 400% of the Federal Poverty Level.

We have locations in all four counties (Lake, Orange, Osceola, and Seminole). Services are available in English, Español, and Kreyòl ayisyen. Our services allow for flexibility and self-determination when it comes to your care. Our case managers assess your needs and provide you the support and resources to access care.
