MEDICAL CASE MANAGEMENT

The Ryan White Medical Case Management program provides HIV-related services for those who do not have sufficient health care coverage or financial resources for coping with HIV.  It is a client-centered service designed to link clients to healthcare in an effort to promote disease management, treatment adherence and improved health outcomes. Medical Case Managers also provide information and referrals for housing, financial assistance, mental health services, substance abuse treatment services, public benefits, insurance, food assistance, transportation, employment, and education.

Clients are assigned to a Miracle of Love Medical Case Manager who work with the client to complete a comprehensive assessment of their needs and personal support systems in order to coordinate access to services. The Medical Case Manager and client develop a personalized service plan to prioritize needs, identify barriers to care and link clients to eligible services and help eliminate barriers.  Once eligibility is conducted by a Referral Specialist, the client is connected to care services.

Linkage to Care is a process in which people testing positive for HIV are assisted in accessing care and treatment for their HIV infection.  This is achieved through a Linkage-to-Care Coordinator and Peer Navigator.  Our goals are to increase the number of people who know their HIV status, remain in treatment on antiretroviral (ARV) therapy, and maintain a suppressed viral load. Counseling and testing programs support people finding out their HIV status.

Medical Case management assistance is also available in Spanish through bilingual staff.

What are the eligibility requirements?

  • You have to be HIV positive.
  • You must be living in Orange, Osceola, Seminole, or Lake County.
  • You cannot be receiving the same services from Medicaid, Project AIDS Care or Insurance.
  • You have to be low-income – at or below 400% of the Federal Poverty Level.

What do I need to bring to my appointment?

In order to be able to quickly get you into the services you need, we will first need to have a few documents to assess your eligibility. Bring the following documents with you to your first appointment:

  • Proof of HIV Status – Original or copy of an HIV Laboratory Test which shows your HIV Status
  • Proof of Income – Bring all that apply to show your income such as:
    • Current Pay Stubs (last 3 months if possible)
    • Military & Earning Statement
    • Rental & Other Income
    • If Self Employed – 1040 Form
    • Public Assistance Award or Denial Letters
  • Bring all documents that show you are receiving services from other programs or have been denied such as:
    • Medicaid Card
    • Project AIDS Care Eligibility Notice
    • Insurance Policy
    • Public Assistance Program Award or Denial Letters
    • Veterans Administration and Other Documents which apply
  • Proof of Residency such as utility bills or lease agreements
  • Picture ID

What If I don’t have all the documentation?

It’s Okay. Bring what you have or call the local office for help. Remember having the correct information at the time of the appointment will speed the process of your eligibility determination.
