POSITION: Peer Navigator (Based in Lake County with some travel to Osceola County)
SALARY: $15.00 per hour plus mileage reimbursement

  • Valid Driver’s License and clean record
  • Background Screening
  • High School Diploma
  • Bilingual Preferred (Spanish recommended)
  • Experience working with PLWHA or having received Ryan White Services is recommended
  • Service consumers living with HIV in Lake County and Osceola County
  • Participate regularly in client case conferences
  • Reinforce treatment plans with clients
  • Assess client’s social needs
  • Promote client utilization of additional clinic services and referrals
  • Teach self-management skills with clients
  • Provide assistance and monitor client progress
  • Educate consumers about safer sex strategies
  • Help clients understand how HIV medications can help them
  • Help clients to make decisions about disclosing their HIV status
  • Educate clients on how to talk openly with their doctors and other providers
  • Serve as a critical bridge between clients and their doctors/providers
  • Meet regularly with Program Managers and receive monthly evaluations (in Lake County)
  • Create an environment that fosters consumer choice
  • Determine access to basic services and explain to clients how to navigate the system as a whole and all available services
  • Provide translation/interpretation services for those with low English proficiency
  • Conduct support groups and facilitate the Consumer Advisory Board meetings in Lake County
  • Enter client updates in the PROVIDE database, as needed
  • Provide status updates to clients’ assigned case manager
  • Participate in monthly Ryan White Team Meeting
  • Attend Quarterly All Staff meeting and other required staff meetings
  • Attend PLWHA Community Meetings
  • Perform any other duties as assigned by Program Manager
  • Physical demands: While performing the duties of this job, the employee is occasionally required to walk, sit; use hands to fingers, handle or feel objects tools, or controls; reach with hands and arms; balance; stoop; talk or hear. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by the job included close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus. Physical or mental ability to carry our the activities of position responsibilities.
  • Work environment: While performing the duties of the job, the employee is exposed to bodily fluids, weather conditions prevalent at the time of duty. The noise level is usually minimal to moderate.
