Project AIDS Care (PAC)
Case Management

The PAC case manager tracks the HIV positive individual client throughout the system of care, assesses needs, develops a plan of care, acts as an advocate, and coordinates services. The case manager will assist in finding service providers and will insure that the services match the client’s needs.  The case manger continually evaluates the safety of the home environment and assists the Registered Nurse with the mandatory annual nurse assessment.

  • Personal Care Services
  • Home Delivered Meals
  • Specialized Personal Services for Foster Children
  • Skilled Nursing Services
  • Massage
  • Consumable Supplements
  • Durable Medical Supplies
  • Education and Support to Clients, their families and their Informal Care-givers

PAC Eligibility Requirement

  • Medicaid eligible
  • Diagnosed with AIDS
  • Disabled
  • At risk of hospitalization
  • Able to be maintained safely in the home
